Any time you can see badly worn spots or if your grips are hard, it's time to either a) change your grips, or b) rotate them.

Rotating them is cheaper (you only pay for labour), but you lose any alignment aids.

On cord grips, the cords will be very flat and very hard.

On other grips, there will be obvious wear spots, and the grips will be hard and slick.
