$118 seems reasonable for a shop price - I do club repairs out of my garage and I would probably charge about $75 for grips and extensions. a 1/2 inch difference isn't that much - you won't notice any change in distance with that little amount of change. You probably won't notice that they are 1/2 inch too short. I would wait until you need your grips changed to add extensions.

As for +2* compared to -2*, if you have noticed that you are taking a heel divot, than yes get them adjusted to what matches you. If you are taking normal divots, leave them at -2*.
