I'll hazard a guess here. Lack of distance and consistency may mean you are moving around, swaying and collapsing your swing.
While the swing is not static, being too ungrounded leads to swaying, losing coil and not staying down and in the shot. Pulling up, reverse pivot, and hitting over the top all come from a poor concept of the swing.
You may want to go to a pro and get him to teach you the fundamentals, take notes and practice them often. Things like the grip, address, weight distribution, and body alignment are easy to practice and will pay off big time in your results.
