hi lee.
At impact your hands should be ahead of the club face. Now, if you are a right handed player, at impact your right hand would be cupped and the back of your left hand should be facing the target . From here you just release the club head into the follow through. the follow through is just a mirror image of your back swing. Try to swing to a full and balanced finish.
Your ball position sounds fine for mid irons.
Woods ,move the ball up in your stance, just inside your left heel.
