You seem to be having two problems: Topping and the ball going right.
They are both caused by the same problem, when this happens you are probably swinging out to in and cutting across the ball with an open club face . Work on bringing the club on an inside out path and squaring the face at impact.
Here is a drill that may help: You may be starting the downswing with your right shoulder and casting the club. At the start of the downswing try to imagine your left shoulder is against a wall and cannot move forwards, your right shoulder should now work down and under your chin, assisting to keep the club on an inside path.
