This shot you transition through to finish were great. Outstanding flexability. (Wish my old beatup body could get there all the time!!!)

Like the other reply, Posture at Address, A few more things are noticeable from this angle. Hands on the takeaway seem to be traveling in a line vice a circle. This movement makes it difficult to cock the wrist naturally. Also your right knee seems to be locked and getting to the extended position a little early in the swing and starting a hair late on the through swing.

Great tempo. Great balance trough this swing.

Your arms are set-up poorly at address. Your left arm is your primary lever and we can see in your address position that it is bent, that means that somehow your going to have to find a way of making the lever shorter again at impact - not very consistant. Your elbows should be set confortably downwards at address so that the left arm can be straight for reasons stated and the right arm can act as a lever without the need to rotate (not very consistant again), anything else but this combination adds unnessesary complications into your swing.

Boy I don't like your takeaway at all - This would be fine if you want to play the LPG system but not CG....lol I would ask you to try this drill to get the right feel of how to take the club back because I don't think you have a clear understanding of what your meant to be doing...

Grip the club on the metal and press the butt end on your left hip area and feel the butt end moves down your leg as you start your turn.... This should combat alot of what your problems are. You can also work on this in conjunction with putting a towel under your arms to really learn to make sure the elements are working together.
