Great turn through impact.
Your setup is with your arms too far away from your body. Stand taller and let them hang straight down.
This will help you get the next problem of the swing plane being flat. You should feel the club over the right shoulder, where you have it quite a but lower then that.
You head does move a bit up and down from takaway to impact... This is a result of your torso being very active in a vertical up and down motion. Work on getting the spine angle constant throughout the swing. This will really help in your ball striking %, not hitting fat and thin shots. You don't see it show up so much on the tee, but your irons in the fairway will see this improvement.
Overall, very powerful compact swing. Good job!
