Not a comment on the swing itself but if the shaft bends like that, perhaps getting a stiffer shaft might be a good idea.....(pause the video just before impact)

I have the same problem as Tim. Basically we uncock our wrists too early (in your case it was your right wrist). In your video, pause that frame which showed the shaft had a massive bent, at this position, the shaft should be parallel (or just under parallel) to the ground. At that point, you uncock the wrist and turn your right side of your body to complete the swing. To help to achieve that, slow down your down swing just a touch more so it gives your body more time to syn with your swing.
Another slight problem was your right leg before impact position. You lifted the heel so high that you may top the ball or not having a good contact. Imagine kicking your right knee towards the ball (ie. towards right and down) and make sure you create a divot after the ball. That's the way to hit iron shots. Good Luck. Tom
