Its sounds to me like a classic case of moving to the next task before the first ones finished.
What you need to do is work on one thing only at the range or the course, just one swing thought will help reduce the stress in your swing.
Your body is perfectly capable of hitting the ball without any interferance from your mind, however when you are working on improvement you need to think about the change until it become engrained into your swing (about three/four weeks), this is how long you need to think about (one) swing thought and do not introduce anymore.
This is what the first sentence is for, by constantly introducing new moves the body becomes confused, for an exercise next time at the range......with the last 20 balls try hitting every single one without any thoughts at all simply hum to yourself when you swing.
This is a great exercise and good practice to help you take your game to the course.
