garys swing

I have some problems. I think, Head moves to much... body Moves too far forward too soon. Left arm breaks near top... Having a hard time nailing down what my swing plane is doing... but it looks strange.

This is the first time I have ever recorded my swing.
I have distance but, want accuracy without totally killing my length off the tee.
Thanks for any pointers.
