Closed clubface at the top of my swing

Been having problems lately hooking the ball and coming over the top. As soon as I hold the club in my hand at address it feels closed and does not feel comfortable at all. After looking at video my clubface is closed at the top of my swing. I never had this problem before is it my grip? Any suggestions? Please help.........
