Severe blocking of tee shots

I play to a 4-handicap and I recently have developed a problem with blocking shots. Most of the time it is from the tee, but occasionally it happens on longer fairway shots. From the tee, it can happen with whatever club I happen to be hitting. It doesn't happen every time, although I am constantly nervous that the problem may happen at any time. For some reason I will get halfway through my downswing, and then my hands will try to keep moving but the clubhead will lag behind as I hang on leading to an awful blocked slice (usually so far right it's either OB or lost). It seems that it has to be mental. I'm having a terrible time trying to stop it from happening. It costs me at least 2-4 strokes per round. Help? Drills?
